Pai Ake Toru is a collaborative partnership between Community Care Trust, Enrich Plus and Community Connections. We offer support for people with an acquired injury.
Pai Ake Toru delivers for ACC clients:
- Facilitated Pathway Map – guided visual exploration of a person’s aspirations.
- Independent Facilitation – identifying community opportunities including cultural, disability, employment and other supports.
- Tailored Supports – meaningful active participation in the community based on the unique goals of a person.
- Coaching to Self-Manage – guided support for a person to determine if they want to manage their own services and supports.

The partnership was created with the aim of delivering high quality services to the people we support over a wider geographical spread whilst keeping the local knowledge and 'flavor'.
By sharing our expertise we are raising the bar for all three organisations, and are achieving more together than we would be able to alone.
